Corine recently joined our staff as a family nurse practitioner. She received her undergraduate degree and Master’s of Science in Nursing from the University of Wisconsin-Milwaukee. Corine has previously worked as a registered nurse in local emergency departments and intensive care units. She sees patients of all ages and enjoys working in Milwaukee’s underserved communities. Corine is a member of the American Nurses Association and the American Academy of Nurse Practitioners. |

Katie is a
Wisconsin licensed mid-level practitioner and certified physician assistant. She earned her master’s in Physician Assistant Studies from Marquette University and has been practicing family medicine for nearly three years. Katie is passionate about providing quality care to her patients and enjoys working in a community health center setting. She also feels that prevention is the best medicine and encourages her patients to get regular check-ups. Katie, a Wisconsin native, is newly married and enjoys photography as a hobby. |

Deanna Graves, CNM

Deanna is a certified nurse midwife and earned her Master’s in Nursing from Marquette University. She provides care to women throughout their pregnancy, including labor and delivery. Deanna is qualified to perform health screenings, diagnose and treat STDs, and discuss the best birth control options for you and your lifestyle. She can also assist in managing the symptoms and progression of menopause. Deanna enjoys partnering with mothers to promote a positive home environment for their babies and is excited to be working in the Hillside neighborhood.

Allison Kos, DO

Dr. Kos is a licensed Wisconsin physician and is board certified in family
medicine. She is committed to treating the whole person, not just symptoms
of an illness. She is a National Health Service Corps Scholar and has
professional affiliations with the American College of Osteopathic Family
Physicians, the American Osteopathic Association and the American
Academy of Family Physicians. |

Van Anh Le, DDS
DENTAL DIRECTOR

Dr. Le completed her education at Marquette University School of Dentistry,
where she currently serves as a part-time associate professor. She is a member
of the Academy of General Dentistry, Omicron Kappa Upsilon dental honors
fraternity and Delta Sigma Delta. Dr. Le specializes in adult dental
procedures and is comfortable treating children, including those under the age of five. She feels that a sense of humor is crucial to providing an atmosphere in
which her patients are relaxed and comfortable. |

Michael Martella, DO

Dr. Martella received his medical degree from the Philadelphia College of Osteopathic Medicine in 2005. He completed his residency at Sister of Charity Hospital in Buffalo, New York and is board certified in Family Practice by the American Osteopathic Association. Dr. Martella is a National Health Service Corps Scholar and is particularly committed to providing healthcare to the underserved. Dr. Martella strives to build strong, trusting relationships with his patients in order to provide each one with personalized care. |

Mehul Sheth, DO

Dr. Sheth earned his Doctor of Osteopathic Medicine from Midwestern University in 2003 and completed his pediatric residency at the University of Illinois in Chicago. He treats children from birth to age 18 and works with parents / caregivers to ensure proper child growth and development. This includes regular checkups, immunizations, lead testing and more. Dr. Sheth is experienced in diagnosing and managing chronic illness, such as asthma and sickle cell anemia. He sees patients at the Hillside Family Health Center during the day and has evening appointments available at the Lisbon Avenue Health Center. |

Khai Ling Tan, MD

Dr. Tan is licensed in obstetrics and gynecology (OB/GYN). She is
qualified to provide care during normal and high-risk pregnancies, in addition to performing gynecological surgeries, routine health
screenings and offering advice about birth control. Dr. Tan believes
that all women deserve quality health care regardless of their age,
race or economic status. She provides her patients with the latest
information and treatment choices, while offering compassion for
intimate health concerns. |

Madelaine Tully, MD

Born and raised in Milwaukee, Dr. Tully received her undergraduate degree from Harvard University. Dr. Tully earned her medical degree from the University of Wisconsin in Madison, where she also completed her internship and residency in family medicine. Dr. Tully provides medical care for all members of the family from birth through older adulthood. She is dedicated to serving the community and has been actively involved in several groundbreaking programs, including the Mosaic Partnership—a social networking project aimed at improving race relations in Milwaukee. Dr. Tully also serves as WHA’s Chief Medical Officer, a role that includes managing the health care providers at both clinics, as well as other administrative duties. |

Nicole Wells, CNM

Nicole is a certified nurse midwife. She has experience working with
patients of all ages and believes in empowering women to make informed
decisions about their bodies. This includes treating the “whole” woman
with respect to physical, social, mental and spiritual factors. Nicole
is qualified to provide care before, during and after pregnancy.
Throughout her career, Nicole has performed and/or assisted in nearly
400 births. She also addresses other women’s health needs through
routine screenings, family planning and preventative medicine. |
